OSU Extension of Defiance County has partnered with Training Achievement Program (TAP) to offer an online version of Ohio Level II Food Safety training which meets all Ohio Department of Health requirements.  [Click on red link above for more information.] 

Fee:  The registration fee is $150 (non-refundable) for both the online class and exam.  There is an additional fee of $25, if you need to reschedule your test date.

Exam: 

• The Food Protection Manager exam is proctored by an OSU Extension Educator who is a Certified Proctor.
• The exam will be online. A computer will be provided.

• May pay with a credit card/debit card (preferred) or with a check. 
• Checks must be mailed to OSU Extension, 06879 Evansport Road, Ste B * Defiance, Ohio 43512.

Participants can complete the online course at their own pace and once training is complete, you will take a proctored certification exam here at OSU Extension Defiance County.
